This form is used by employers to remit SIMPLE IRA contributions to financial organizations for credit to the SIMPLE IRAs of the employees participating in the SIMPLE IRA plan. It provides a breakdown of the contribution for each participant, including identifying information such as the employee's Social Security and account number. A place for the employer to designate the type of contribution (employee elective deferral, employer matching, or employer nonelective) is also provided. This form may be used for SIMPLE IRA Plans established with either the 5304- or 5305-SIMPLE Plan Snap-Pac. Three-part, one-sided, black ink, 8 1/2 x 11.
